The Medical Home Community Team provides home-visiting and care coordination services for families to help address social determinants of health and development. MHCT offers assistance to Philadelphia County parents and their children, with and without special healthcare needs, ages 0-21 years old.
Services include: comprehensive needs assessment, individualized health education, referrals and linkages to behavioral health, transition to adult medical care support, community organizations and service coordination in conjunction with partnered medical homes.
