Diabetes is a disease in which blood glucose levels are above normal because the body either doesn't make enough insulin or can't use its own insulin as well as it should. Diabetes can cause serious health complications including heart disease, blindness, kidney failure, and lower-extremity amputations. HPC programs address diabetes in all communities, but focus specifically on two disparately affected populations, tobacco users and the African American community through education, healthcare provider training and patient outreach.
